2022 Church/Group Registration Form

This form is due to the Fusion Camp Office by end of April. Once filled out and submitted online, please send a check to the Fusion Camp office for $35.00 or $50.00 depending on the size of your youth group that will be attending camp.

Church/Group Application Deposit

$35 for groups of 9 or less (students and adults), $50 for groups of 10 or more (students and adults): Due by end of April along with this form.

Guesstimate of how many you will have attending camp *

There will be a deposit of $100 per person (both students and adults) due by end of April to insure your place at Fusion.
Per person deposits are credited towards the balance due for camp.

Your registration is not complete until your church registration check has been received at the Fusion Camp Office.
